An innovative program helping at-risk patients get the medical care and access they need

Health Connections has helps people sign up for health insurance, transportation to the doctor's office, financial help with prescriptions and receive medical supplies that they otherwise couldn't afford.

Highline Medical Center Foundation has partnered with Highline Medical Center to create Health Connections - a program made to help members of the community take steps towards healthier lives.

SEATTLE — Highline Medical Center Foundation helps fund Health Connections, a program to help members of the community take steps toward healthier lives.

"The program is intended to help vulnerable patients who tend to use the emergency room a lot and may not have a primary care provider," said Doctor Nicholas Jansson from Highline Medical Center.

The care team at Health Connections addresses the patient’s needs (medical, mental health, substance abuse, social services, housing, transportation, food, low healthcare literacy, etc.) in an integrated, holistic manner to better serve the patient.

"The program has a very focused mission to really personalize at-risk patients healthcare," said Jansson.

Delores is a Health Connections patient that has used their services since she got out of the hospital in April.

"The second day I got out of the hospital my doctor referred a physician from Health Connections to me and it was just like magic, that very same day she began to help me with at-home care,"

The physician from Health Connections also arranged for Delores to get medical care in the comfort of her own home.

The areas of coverage for Highline Health Connections include Burien, Des Moines, SeaTac, Federal Way, Auburn, and White Center.
